The purpose of this page is
to facilitate the process of submitting contacts active in your
local to the Clearinghouse, for record keeping and other matters
such as determining the votes alloted to your local at meetings
of the Assembly.

form will decrease the time demands and frustration of the Clearinghoue
Coordinator and Database Coordinator by providing a printed
list to check sign-in sheets against.
• Using this
form will help ensure that the members of your local are properly
accounted for, and that their names are spelled correctly
in our records.
